NHS SomersetICB is seeking an experienced HCPC registered Occupational Therapist to join the busy All Age Continuing Care/CHC team, which ispart of the wider Quality and Nursing directorate.
You will be an expert practitionerin occupational therapy practice and provide clinical expertise, specialistadvice for adults, children and young people with complex needs who requireassessment and equipment provision within the NHS Continuing Healthcare (CHC)and Children and Young People Continuing Care (CYP CC) criteria.
You will need excellent assessment andinterpersonal skills, and be passionate about supportingindividuals with long-term complex needs, experience in All Age ContinuingCare is desirable but not essential.
You will need excellent organisational skillsand a sound, knowledge of MS office packages such as Word, Excel, Outlook,Excel etc., alongside excellent written and verbal communication skills bothvirtually and in person.

Main duties of the job
* To act as an expert practitioner for the wider AACC team, other occupational therapy staff, other clinical and social care colleagues in respect of the complex clinical needs of adults, children and young people who meet the CHC/CYP CC criteria.
Undertake assessments for individuals with an identified Occupational Therapy need including adults, children and young people who live in residential nursing homes and specialist placements both in and out of county as well as for individuals in receipt of Personal Health Budgets (PHBs) in their own homes and supported living accommodation across Somerset.
Make decisions on NHS Continuing Healthcare eligibility for adults as part of a Multi-Disciplinary Team (MDT) approach
* To be able to move and handle adults, children and young people with complex physical problems and disabilities, assess, prescribe and adjust specialist equipment.
To provide expertise to CHC/ CYP CC panels and to the wider AACC team in relation to CHC applications and care provision

Tocontribute to providing first level contact for disputes and local resolutionsbetween partner agencies/or individuals or legal bodies

Undertake and support auditrequirements relating to CHC/CYPCC
To support the development ofand implement local policies, systems and processes around CHC/CYP CC, linkedto the relevant key national and local priorities
To promote excellent customercare throughout the CHC/CYP CC process ensuring local resolution of anyconcerns where possible. This willinclude providing and receiving highly complex, sensitive and contentiousinformation requiring agreement or co-operation where there are barriers tounderstanding. This relates to externalorganisations and the public as well as to staff of the ICB and local authority
Tosupport service requirements, undertake training and assessments relating toDeprivation of Liberty Safeguards (DoLS)
Ensure effective timelycommunication and liaison with key stakeholders, including partner agencies,Local Authorities, GPs, individuals andtheir families, and care providers
Working with AACC seniormanagers and cascade information as appropriate
Investigating and respondingto any complaints
Provision of expert clinical(within the OT remit) and non-clinical advice in relation to CHC/CYP CC
Working with discharge teamsto promote timely discharges and support understanding of CHC/ CYP CC as partof the discharge process
Maintaining expert knowledgeof county-wide OT services including public and private sector provision
Undertaking necessarytraining
Identifying priorities withinown workload. Devise and implementprogrammes to manage workload demands
Leading in projects, pilots,research related to CHC/CYP CC, for example in response to specific requestsfor local data from the Department of Health and provide comprehensive reportsto Service Lead.
Raising safeguarding concernswhere appropriate and working with Safeguarding leads in support of CHC/ CYP CCfunded individuals
Support quality improvementin relation to the services provided to CHC/ CYP CC funded individuals
Attending case conferences/Best Interest meetings/Court of Protection/ Child in Need meetings as necessary
Contributing to thedevelopment of contracts and commissioning arrangements for packages of careand work with contract and commissioning managers to ensure that care deliveredto CHC/CYP CC funded individuals is safe and effective through clinical qualitycontract review processes
