The successful candidates will need to be passionate about delivering cancer care services within a primary care setting, improving uptake of cancer screening and acting as a first point of contact for cancer patients. They should feel comfortable discussing emotional and difficult conversations daily in both a telephone and face-to-face capacity. Candidates will be caring, dedicated and have excellent communication skills with the key ability to listen to the patients' needs. Excellent written and verbal communication skills are required, along with strong organisational and time management skills.
Job Description and Responsibilities:
1. Ensure all cancer patients receive the appropriate information, onward referrals and support as needed to meet the requirements of the DES for Early Cancer Diagnosis in Primary Care as required by the NHS Long Term Plan.
2. Be a key point of contact for newly diagnosed cancer patients.
3. Be a key point of contact for patients referred via the suspected cancer referral pathway from primary care into secondary care.
4. Communicate with patients by letter, telephone or face-to-face appointments to ensure they receive relevant supporting information, including the importance of their attendance at hospital appointments.
5. Manage patients' needs appropriately, documenting all consultations within the patients' medical notes using appropriate systems.
6. Work with patients, families, and carers to provide support and care and manage their needs.
7. Contact, organise and complete the cancer care review for all patients with a new diagnosis, offering a GP appointment where appropriate.
8. Complete a Cancer care review with patients via telephone consultation or face-to-face, recording an accurate and concise consultation within the patients' notes using EMIS.
9. Make appropriate referrals from the completed Cancer care review, recording within the patient notes and complying with relevant data privacy and consent.
10. Liaise with appropriate GPs and professionals to maximise patient needs, including identifying patients for the Gold Standard Framework (GSF) list and flagging patients with Out of Hours support.
11. Discuss benefits and allowances, assisting in ensuring Respect forms are in place.
12. Encourage and assist people to self-manage their health and well-being, increasing their knowledge, skills, and confidence in managing their health, linking with other personalized care roles across the PCN Network.
13. Build effective relationships with practice staff and local system partners such as the Hospice Outreach team, District Nurses, Public Health, and Cancer Alliances.
14. Work effectively both within a team and independently.
15. Gather information and data when requested to prepare reports and documents as requested by PCN leads.
16. Undertake data analysis to support the increase of cancer screening prevalence throughout the PCN.
17. Create, distribute and maintain health promotion material and patient information within practices.
