About The Role
Are you ready to make a real difference in people's lives? Do you have a passion for helping others stay well and out of hospital? If so, we invite you to apply to join our Individual Budget Support team in Cornwall as a part-time Support Worker, working in the Newquay/Bodmin area.
The Individual Budget Service is dedicated to supporting customers with severe and enduring mental health diagnoses, helping them achieve a better quality of life through social inclusion. We work closely with specialist providers to deliver proactive, holistic support, empowering our customers to build confidence, self-esteem, and pride, and to move towards greater independence.
Key Responsibilities:
* Personal Support Plans: Provide tailored support based on each customer's Personal Support Plan, focusing on mental health and other areas such as tenancy management, benefits, education, training, and volunteering.
* Community Engagement: Help customers access local services and become active members of their community.
* Health and Wellbeing: Support customers with health needs, addiction issues, and liaise with other professionals to ensure comprehensive care.
* Referral Partnerships: Regularly liaise with local Mental Health Teams and Adult Social Care in Cornwall Council to assess needs and risks, ensuring the service remains suitable for customers.
* Caseload Management: Maintain up-to-date caseload management systems, complete and review Risk Assessments and Support Plans regularly.
We seek motivated individuals who are eager to make a positive impact on the lives of those we support. Experience in a similar role is desirable, but the most important quality is a genuine passion for helping people in the local community.
This role can be challenging, as you will work with and support people facing a range of difficult issues and behaviours. Strong teamwork skills, the ability to work independently, enthusiasm for collaborating with partner agencies, resilience, attention to detail, initiative, and problem-solving abilities are essential.
This is a field-based role, with travel between sites; therefore, access to a suitable vehicle is essential.
