Clinical Nurse Specialist (Hospice at Home)
Rotherham Hospice is seeking a compassionate and experienced Clinical Nurse Specialist to join our Hospice at Home Service Team.
Reporting to the Head of Hospice at Home, the Clinical Nurse Specialist plays a crucial role in delivering high-quality palliative and end-of-life care to patients in their own homes.
With a focus on holistic and person-centered care, the Clinical Nurse Specialist provides expert clinical support, assessment, and coordination of care, working collaboratively with patients, families, and multidisciplinary teams to ensure the best possible outcomes.
2 or 3 x 12.5 hours, Rota weekends bank holidays, may need to do nights.
Main duties of the job
* Provide expert clinical assessment, monitoring, and management of complex palliative care needs for patients receiving care in their own homes.
* Develop and implement individualised care plans in collaboration with patients, families, and other healthcare professionals, ensuring holistic support and symptom management.
* Offer emotional support, education, and guidance to patients and their families, empowering them to make informed decisions about their care and treatment preferences.
* Act as a key point of contact for patients and families, providing ongoing communication, reassurance, and advocacy throughout their care journey.
* Coordinate care delivery with members of the Hospice at Home interdisciplinary team.
* Collaborate with community healthcare providers, primary care teams, and specialist services to optimize support and resources for patients and families in the home setting.
* Provide supervision and guidance for volunteers, working with the Volunteer Manager when necessary to maximise volunteer impact.
* Provide education and training to patients, families, and caregivers on symptom management, medication administration, and other aspects of palliative and end-of-life care.
* Maintain accurate and timely documentation of patient assessments, care plans, interventions in accordance with hospice policies and regulatory standards.
* Participate in ongoing evaluation and quality improvement activities, identifying areas for enhancement to improve the delivery of hospice at home services.

Person Specification
Experience
* Extensive experience in palliative and end-of-life care, including clinical assessment, symptom management, and coordination of care in the community or home setting.
* Managing palliative care patients in crisis and end of life.
* Experience working with SystemOne.
* Experience working with volunteers.

* Strong clinical assessment and decision-making skills, with the ability to provide expert clinical support and guidance to patients and families.
* Excellent communication and interpersonal skills, with the ability to build rapport and establish trusting relationships with patients, families, and multidisciplinary teams.

* Multilingual proficiency is highly desirable as it enables effective communication and engagement with a diverse range of stakeholders, including patients, families, and staff from multicultural backgrounds.
Qualifications
* Registered Nurse with current registration with the Nursing and Midwifery Council (NMC).
* Advanced qualification in palliative care or related field (e.g., Palliative Care Nursing, Advanced Nurse Practitioner).

£39,795 to £48,639 a year depending on experience.
