The postholder will ensure that medications are used safely, effectively, and cost-effectively for all Trust patients. This is a rotational position, primarily based at the Denmark Hill facility.
The post holder will be managing a Medicines Management Pharmacy Assistant (MMPA) and if necessary, another junior staff.
Main duties of the job
* To provide a specialist patient-focused technical pharmacy service.
* To counsel patients and/or their carers on their medication.
* To assess suitability of patients’ medicines on admission and discharge.
* To provide appropriate and sufficient supplies of medication during admission and on discharge.
* To facilitate discharge through the timely and efficient supply of medicines.
* To help with Control Drug returns.
* To liaise with ward and pharmacy staff to facilitate the efficient transfer of medication and relevant information when patients move wards.
* To follow through any relevant queries arising from patient transfer or discharge.
* To liaise with pharmacists, ward staff, general practitioners, community pharmacists and other primary healthcare professionals as required to resolve queries and ensure accurate patient medication histories are obtained, in line with local policy.
* In liaison with pharmacists, to obtain and document patient medication histories, after appropriate training and/or accreditation.
* To support with individual patient medication management issues, within own skills and expertise.

Detailed job description and main responsibilities
This role will be integral in supporting the provision of clinical services to patients on the wards. This role will involve the following:
* Ward based dispensing services to streamline patient discharges.
* Participation in an innovative 7-day pharmacy service.
* Engaging and working in collaboration with a broad multi-disciplinary team.
* Ensuring medication storage and security criteria are met to comply with national standards.
* Gaining an underpinning knowledge and practical experience of performing medicines management duties at ward level.
* Rotating through a wide range of specialties including acute medicine, critical care, neurology, paediatrics, cardiology, renal, haematology and surgical/gastro medicine.

Person specification
Registration
* NVQ Level 3 plus accredited underpinning knowledge [BTEC in Pharmaceutical Science or equivalent] or equivalent.
* Registered as a pharmacy technician with the General Pharmaceutical Council (GPhC)
* Accredited Checking Pharmacy Technician (ACPT) qualification or working towards.
* Mandatory CPD to maintain fitness to practice
* Successfully completion of development courses relevant to practice
* HEE Medicines Optimisation course or equivalent (or to be achieved within 6 months of employment)
Experience
* Knowledge of all regulations and legislation relevant to pharmacy, dispensary and clinical services
* Knowledge of pharmacy computer systems
* Substantial post registration experience in hospital setting
* Experience of training and supervision of pharmacy staff
* Experience of delivery service to patients or the public.
* Experience of research and audit work
Skills
