Are you a registered nurse, allied healthcare or social care professional looking to build on your management and leadership experience in an integrated health and social care setting? Whether you are a nurse, physio, OT, paramedic practitioner or social worker, you will use your expertise to lead a team to facilitate the seamless discharges back to the patients' home or into the community. You will work closely with the medical and nursing teams to ensure patients are discharged as soon as they are medically fit and will be key in supporting the Trust's length of stay and patient flow programme.
The ideal candidate will be able to demonstrate a good knowledge of both community and acute healthcare settings, have an understanding of discharge planning relating to a range of medical and surgical conditions, and have the ability to monitor preventable delays in the discharge process.
The applicant will need to be able to innovate and champion best practice and provide inclusive, compassionate leadership and operational management.
