Job summary The post holder will support the Barking and Dagenham Place Based Partnership, and be a subject matter expert in the delivery of a number of its core priorities and outcomes. The post holder will have good knowledge of the policy and guidance related to their dedicated area, with a strong understanding of and experience in the management and co-ordination of multi-agency meetings. The postholder will require good communications skills and be able to work sensitively with a range of stakeholders across health, care and the private sector and with children and young people and their families to co-ordinate the function, often working to tight timelines. The postholder will be expected to work as part of a team of CETR/CTR co-ordinators across BHR to provide cross cover when needed and work with colleagues in other Place Partnerships to share best practice and learning. Main duties of the job The remit includes (but is not limited to) the following areas within the Place:o Leading on the co-ordination, reporting and monitoring of CETR meetings and reviews (online and face to face) within the timescales required. Some face to face meetings may require travelling out of the borough.o Leading on the coordination of professional meetings and Local Area Emergency protocol (LAEP) meetings.o Co-ordinating monthly assurance meetings with the Local Authority and provide support to the maintenance of the Dynamic Support Register.o Maintaining a CETR tracker including a tracker for discharge plans which includes S117 applications, capacity assessments, risk assessments and barriers to discharge.o Leading on a KLOE tracker and report.o Supporting the completion of Root Cause Analyses to identify where improvements to care can be made.
