To work as an integrated member of the Urgent Community Response Service delivering person centred care, assisting Occupational Therapists, Physiotherapists, Nurses, Paramedics and the wider multidisciplinary team with patient interventions. To deliver clinical interventions defined within individualised patient care plans, as directed by the senior staff. To maintain high quality standards in all aspects of care delivery, working in partnership with primary care colleagues, other health professionals, patients, carers and the statutory, independent and voluntary organisations.
