Pier Health has an exciting opportunity for hardworking and enthusiastic nurses and HealthCare Assistants to join our friendly Primary Care Network team.
Main duties of the job
We are looking to grow and expand our current offer to include:
1. Adult Immunisations including Covid & Flu
2. NHS Health Checks
3. Learning Disability & SMI Annual Reviews
4. Annual reviews for Asthma, COPD, Diabetes
5. Cervical Cytology
Clinical activities will be held in a variety of locations not always GP practices. We also visit patients in their own homes as part of our vaccination programme.

About us
We are growing our clinical and non-clinical teams to wrap additional support around our GP practices. With mental health nurses/assistants, dietetics, social prescribing, frailty liaison and enhanced Care Home support, on-site pharmacy, paramedic homebound support, first contact physiotherapy and some brilliant community partners across our patch, we are excited to see that by reducing red tape and working in a truly multi-disciplinary way, we are reducing GP workload while improving outcomes for our patients.

Experience in Primary Care is desirable but not essential.
Job responsibilities
The role is to deliver high quality patient care to meet the holistic health care needs of practice populations. The post holder will be suitably trained and be competent in at least two of the various activities detailed in key responsibilities.
Key Responsibilities
1. Participate in vaccination and immunisation programmes for both adults and children- including vaccinations for those travelling abroad.
2. Provide wound care (ulcer /Doppler etc.) to patients.
3. Taking patient samples, swabs, blood, specimens, and checking pulse, temperature and blood pressure.
4. Support and advise women requesting information relating to family planning needs and those presenting for cervical cytology consultations.
5. Give advice, education and information about chronic health conditions and ailments, smoking cessation and losing weight.
6. Document all patient contacts in the electronic patient record.
7. Ensure all training is up to date.
Working Conditions
You would be required to work in several different environments across the Primary Care Network.
Training and Development
All roles within Pier Xtra have an element of statutory mandatory e-learning training which will be completed within the first two weeks of employment. Any extra training required for role development will be provided as required.

* Good interpersonal Skills
* Good level of communication both verbally and in writing

Experience
* Working inter-professionally
