The Waterfront Medical Centre is a well-organised, motivated and friendly training Practice based in Barry. We currently have a list size of approximately 12,500 patients. The Practice iscommitted to high quality patient care along with the continued development of our team. We are a long established Practice with a strong emphasis on team work and having a good work/life balance. We are a forward thinking practice which have always encouraged innovationandhaveconsistently been a high QAIF achiever. We also offer a range of Enhanced services to patients including Steroid injections, Coils and Implants etc. The practice has recently started using AskMyGP which has been positively received by both patients and staff. The Practice is part of the Central Vale Cluster, consisting of 7 Practices in total.We work collaboratively as part of the Cluster to develop new projects which are specifically tailored to benefit the area's patient population. The Cluster currently provideFirst Contact physiotherapy, Pessary Services, Mental Health Specialist Nurses and Chronic Wound Care services. The cluster also holds a virtual weekly MDT meeting which primarily focuses on thosepatients who require a multidisciplinary team approach. Main duties of the job Clinical Responsibilities: The post-holder will make him/herself available to undertake a variety of duties including surgery Consultations, telephone consultations and queries, triage calls, triage visits, checking and signing of repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion. Assessing the health care needs of patients with undifferentiated and undiagnosed problems. Screening patients for disease risk factors and early signs of illness. In consultation with patients and in line with current Practice disease management protocols, developing care plans for health To assist in the establishment of appropriate systems to manage common chronic medical conditions. Completion of clinically related administrative and non clinical duties needed for the delivery of the service.
