Then we have the job for you We are looking for a Rehabilitation Assistant to join the Proactive Care Team, working with Community Matrons and Physiotherapist in First Community Health and Care. The Rehabilitation Assistant works collaboratively within the team, providing care and rehabilitation to patients at risk of being admitted to hospital. The post of Rehabilitation Assistant involves providing care visits and rehabilitation in the patients own home under the direction of the Community Matrons and Physiotherapist. Due to nature of the role, it is essential that you have a full UK driving licence, access to a car and insurance that covers you for business use. All staff undertake an induction programme. At First Community Health and Care, training and development opportunities are supported through an annual appraisal process and in-house training. The service operates from 8.30 4.30 Monday to Friday (not including bank holidays).
