Old Station Surgery is looking for an enthusiastic, hardworking and motivated Advanced Nurse Practitioner to join our expanding team.
The successful applicant will be joining an innovative and experienced team, contributing to the delivery of high standards of clinical care across our surgery.
Within this role you will be required to work independently, making professional decisions and referrals, where required. You will be responsible for history taking, clinical assessment, diagnosis and treatment plans within the scope of your role.
Main duties of the job
An Advanced Nurse Practitioner at Old Station Surgery will be an experienced practitioner who, acting within their professional boundaries, will provide care for patients presenting at the practice from initial history taking, clinical assessment, diagnosis, treatment and evaluations of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing leadership as required.

The following are the core responsibilities of the ANP. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
6. Work as an autonomous practitioner and as part of a multi-disciplinary team in order to ensure patient needs are met.
7. Managing patients presenting with a range of acute and chronic medical conditions, providing subject manner expert advice to patients via telephone triage and in person consultation.
8. Supplementary prescriber, adhering to guidance and working within own scope of practice.
9. Request pathology services as necessary.
10. Process and interpret pathology and other test results as required.
11. Maintain accurate clinical records in conjunction with extant legislation.
12. Ensure read codes are used effectively.
13. Prioritise health issues and intervene appropriately.
14. Support the team in dealing with clinical emergencies.
15. Recognise, assess and refer patients presenting with mental health needs.
16. Extend and supplementary prescriber, adhering to extant guidance.
17. Support patients in the use of their prescribed medicines or over the counter medicines ,within own scope of practice, reviewing annually as required.
18. Contribute to practice targets complying with local and regional guidance.
19. Liaise with external services / agencies to ensure the patient is supported appropriately -vulnerable patients etc.
20. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual).
21. Support the clinical team with all safeguarding matters, in accordance with local and national policies.
22. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.
23. Deliver opportunistic health promotion where appropriate.
In addition to the primary responsibilities, the ANP may be requested to:
24. Participate in audits as required.
25. Acting as a mentor to students and newly qualified staff.
26. Support and participate in shared learning within the practice.
27. Participate in the review of significant and near-miss events applying a structured approach, root cause analysis - RCA.
28. Maintain professional registration and adhere to re validation requirements.
