Job summary We are seeking an experienced Band 5 Learning Disability Nurse looking to progress into a Band 6 Community role, or an existing Band 6 Nurse, to join our dynamic and supportive team working with adults with learning disabilities in the City & South of Oxfordshire. This role offers a diverse opportunity, covering both physical and mental health needs. You will work within a Multi-Disciplinary Team (MDT) alongside a range of health professionals, supporting individuals with learning disabilities in North and West Oxfordshire. We are looking for a Registered Nurse (Learning Disabilities) to join us on a permanent full-time basis, starting as soon as possible. You'll play an integral role in delivering person-centred care, supporting individuals to improve their overall health and wellbeing. As our Senior Community Learning Disability Nurse you'll play a key role in delivering care for adults with learning disabilities, collaborating within an MDT to create care plans and supporting patients with their physical and mental health needs. Main duties of the job You will report directly to the Locality Lead Nurse and will be responsible for providing leadership and management to nursing and healthcare support staff within their designated area of work at Oxford Health Foundation Trust. In collaboration with the multidisciplinary team (MDT), they will contribute to care delivery, taking a lead on specific areas. This will be done within the available resources. As a Senior Community Learning Disability Nurse, the role involves working as part of the MDT to assess, plan, implement, and evaluate the healthcare needs of adults with learning disabilities, following the framework of: MDT collaboration principles The holistic care model (Four-Tier model) Seven areas of clinical practice Key responsibilities include: Managing a complex caseload of patients with learning disabilities, being accountable for conducting comprehensive nursing assessments, creating care plans, and implementing nursing interventions. This includes developing a variety of care plans (e.g., Positive Behaviour Support, epilepsy care) and ensuring the inclusion of patients' and carers' views in these evidence-based plans.
