Job Advert Role Description
Glebe House (formerly part of Craegmoor) is a Residential Home providing support for up to 24 adults living with a primary Mental Health diagnosis and possible other complex conditions
Some individuals also have difficulty with communication and present behaviours that challenge, as well as a range of other conditions such as epilepsy or learning disability. Some of the individuals with mental health issues may self-harm and have complex personality disorders, depression, bipolar disorder or other conditions that they need support with.
Previous experience of working with adults with Mental ill health and/or behaviours that challenge would be beneficial for this position.
Team Leader Duties
• We are looking for a Team Leader to assist the Manager and General Manager of the Service to work as part of a multidisciplinary teams.
• Organises & co-ordinates and the delivery of the Service’s care programme
• Conducts the welcoming of new residents ensuring they are comfortable and familiar with the environment and to impart essential information to their families and or carers.
• Ensuring service user files and records are updated in an accurate and timely manner.
• Observe and monitor the wellbeing of the residents ensuring any unusual physical, mental or emotional occurrences are promptly referred to more senior staff and documented as appropriate.
• Participate in the out of hours on call rota and respond as required
• Supervises support workers in delivery and supports people who use our service in achieving their outcomes in the way that they choose.
• Support staff in own team to maintain effective professional relationships with the people who use the service, their families, colleagues, outside agencies and other professionals referring any issues to senior staff as appropriate.
