Job overview
An exciting opportunity has arisen for a Band 6 Community Practitioner to work within the North Tyneside Recovery Partnership. The post holder would be working into our Physical Health pathway.
The role entails working with those affected by substance and alcohol misuse providing safe and effective interventions, harm reduction advice, and use of psycho social interventions to support service users to consider and achieve recovery.
The team consists of a well-established mix of professionals that work collaboratively to deliver individualised care. The post holder will work as a member of the Multi-disciplinary Team.
Main duties of the job
* Work directly with those who have a physical health need, potentially supporting service users to other health care settings.
* Taking general height, weight and physical health observations.
* Communicating with GP's, District Nurses, Hospitals, and other Health Care professionals.
* Potentially require some wound care management.
* Provide and assist in blood borne virus assessment & treatment pathway.
* Conduct individualised clinical assessment, care planning, formulation, and collaborative working across the multi-agency team.
* Offer specialist advice and information to colleagues around physical health.
* Demonstrate leadership within the team, providing supervision and supporting students.
* Responsible for the Assessment and Management of casework of service users who have substantial and complex needs.
* In collaboration with the Service User and Carer, develop Recovery and Wellbeing focused Care Plans, ensuring regular reviews to monitor progress.
* Consider Carer needs and ensure Carer and family are involved where appropriate.
* Deliver evidence-based Clinical and Psychologically based Interventions.
* Coordinate Treatment Planning & Interventions, Reviews, and Discharge Planning.
Person specification
Education and Qualification
* Professional Qualification and current Professional Registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist.
* Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
* Post-registration clinical experience relevant to the post.
* Appropriate training (e.g., Safeguarding, MAPPA, Child Protection).
* Up-to-date knowledge/experience of relevant legislation.
* Experience of working within a community team.
* Non-Medical Prescribing.
Knowledge and Experience
* Awareness and understanding of professional accountability, NMC standards, and other local and national initiatives.
* 12 to 24 month band 5 qualified nurse experience dependent upon service need.
* Understanding of difficult to manage/challenging behaviours.
* Experience and knowledge of working within drug and alcohol.
Skills and Competencies
* Ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs, including Risk Assessment and Risk Management/Safety Planning skills.
* Demonstrate an understanding of effective & appropriate treatment options including a range of evidence-based clinical approaches as identified in N.I.C.E. Guidance.
* Demonstrate effective written and verbal communication skills. Evidence of a high level of professional, organisational and leadership skills (self & others).
* Ability to adapt within a changing environment and work under pressure.
* Leadership & Delegation Skills.
Additional Requirements
* Ability to meet travelling within the geographical location and potential surrounding areas.
