Your Opportunity
This position is responsible for conducting assessments for older Australians to determine eligibility for government funded aged care services. The role is essential in the delivery of personalised, client centered assessments that promote independence and improve quality of life. The aged care assessor ensures that all assessments are completed in accordance with the Aged Care Assessment Quality Framework.
Key Responsibilities
* Conduct detailed, face-to-face assessments for clients seeking aged care services
* Observe clients in their home or community settings to gain insights into their daily routines, environmental risks, and overall needs;
* Collaborate with clients, their families, and representatives to develop tailored support plans based on individual needs, preferences, and goals;
* Ensure all assessments and support plans adhere to the Aged Care Assessment Quality Framework and best practices in clinical governance;
* Engage with clients and their representatives to explain the assessment process, service options, and available supports;
* Facilitate referrals to approved service providers based on the client’s assessed needs and approved support plan.
Desired Skills and Experience
* Relevant qualifications in Aged Care, Social Work, or a related field;
* Experience working in Community Services, Aged Care, or Human Services
* Current C class Driver’s license (QLD).
* Proven experience in conducting comprehensive assessments within Aged Care, Disability, Health, or Community Services environments.
* High level of interpersonal, communication and negotiation skills, particularly when working with clients, families and multi-disciplinary teams;
* Demonstrated ability to manage a caseload of clients, prioritise tasks and meet deadlines in a high-volume work environment;
* Sound level of cultural competency, with the ability to provide services to clients from diverse backgrounds;
* Sound level of practical experience in the use of applicable corporate systems and client management systems, including the Microsoft Office Suite.
Other requirements:
* Working with Children Clearance (Blue Card) and NDIS worker screening (Yellow Card)
* National Police Check
