We are currently looking for a Senior Physiotherapist to join our Intermediate Care Team in the North, based in Peterborough. The vision of the service is to provide outstanding, patient-focused, community-based health care and therapy, optimising people's potential to achieve their goals by supporting earlier discharge from hospital using the home first model.

Main duties of the job
* To provide comprehensive, holistic therapy assessment and rehabilitation to patients within their home environment, maximising independence within a 4-week pathway.
* To hold clinical responsibility for patients within the service and Therapy Practitioners within their team. To prioritise workload including that of the Therapy Assistants within the team.
* To work proactively, in collaboration with the wider multidisciplinary team to ensure patient flow and safety.
* To assist in the development of the Integrated Care Workers and Therapy Practitioners to support the clinical delivery of the service.
* Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence and have use of a vehicle.

Person specification
Education / Qualifications
* Degree or equivalent eg. BSc, MSc or Diploma entry level qualification into the Physiotherapy profession
* Current HCPC registration
* Documented evidence of own continuing professional development
* Willingness to undertake Fieldwork/Practice Educator qualification
* Member of CSP
Experience
* Relevant post-graduate clinical experience in a variety of settings
* Clinical supervisor experience
* Community experience
