Bournemouth East Collaborative Primary Care Network (PCN), situated on the stunning Dorset South Coast, comprises four like-minded practices working together in East Bournemouth, with a strong reputation on quality improvement and investing in its employees. The PCN serves a population of approximately 55,000 patients with a diverse demographic.

* Enhanced Care Visiting team for frail housebound patients and those in care homes.
* BEC Urgent Care Treatment Centre working out of the Treatment Centre at Shelley Manor Medical Centre.

* Digital Lead, Digital Care Coordinator, Digital Champions.
* Pharmacy team comprising of Clinical Pharmacists, Pharmacy Technicians and Care Co-ordinator.

Bournemouth East Collaborative Primary Care Network (BEC PCN) has a new and exciting opportunity for an ANP/ACPs to join the BEC Treatment Centre Team.
We are looking for ANP/ACPs to work in hours and out of hours service at the PCN Treatment Centre. The service runs Monday to Thursday 4pm-8pm, plus Saturdays 9am-5pm.
The sessions we are looking for:
Monday and Wednesday 8am to 4pm, and Thursday 4pm to 8pm.
Salary: From £26.07- £27.48 per hour (In-hours/Depending on experience), £33.89-35.72 per hour (OOH – 6.30pm-8pm and Saturdays/ Depending on experience)
This role will require an Enhanced DBS Clearance. Should you have this certificate as part of the DBS subscription service, we would be happy to accept this providing the original is shown. Otherwise, you will be expected to complete a new application.
Job summary
The Treatment Centre is based at Shelley Manor Medical Centre, comprising of multi-disciplinary healthcare professionals working towards a vision of providing integrated care for all our patients and improving access.
ANP/ACPs will be expected to provide assessment, diagnosis, and treatment to same day face to face appointments. If appropriate prescribe and refer patients to appropriate care providers, within locally agreed PCN healthcare pathways and complete 2 week waits if required.
Main duties of the job
* Autonomously manage own appointment list of patients across the age continuum to provide whole episode of healthcare by seeing, assessing, diagnosing, treating and discharging or referring patients attending the Treatment Centre across a wide range of undifferentiated and undiagnosed conditions including minor injuries and minor ailments.
* Able to use an advanced level of professional accountability, autonomy and judgement for seeing treating and assessing patients who present with a broad range of situations and conditions.
* Plan and manage complete episodes of care, working in partnership with other members of the multi-disciplinary PCN team, delegating and referring as appropriate, to optimise health outcomes for patients.
* Work autonomously, clinically examining and assessing patients and initiating any appropriate action whilst taking responsibility for managing practice whilst using an advanced level of professional accountability, autonomy and judgment for highly complex decision making across a broad range of situations.
* Have a level of understanding of PCN and NHS values, policies and procedures.
What we need from you:
Essential
* Professional registration with relevant professional body.
* Recognised Advanced Clinical Practice course (or working towards) – MSc or Advanced Practice Digital badge.
* Non-medical prescriber.
* Ability to see and treat a wide range of minor injury and illness presentations.
* To take a full comprehensive history, clinically physically examine and treat within level of own competence.
* Willing to learn clinical new skills to enhance practice.
* Evidence of on-going CPD.
Desirable
* Interpretation of X-rays.
* Understanding of the Primary Care Network.
* Additional skills of assessing elderly care.
* Teaching and developing staff.
* Ability to analyse and interpret complex information/results from patient assessment.
Patient/Client Care
* Takes responsibility for providing care and treatment in line with current evidence base practice at an advanced clinical practice level.
* Demonstrates the ability to make critical judgements and utilise problem-solving skills, regarding patient care, on a daily basis.
* Initiates and carries out highly complex patient assessment, analyses and interprets results, using highly developed advanced knowledge and skills.
* Independently prescribes a range of medications, as agreed at PCN level, based on self-competencies, efficacy, safety and cost.
* Discusses and agrees assessment outcomes with patients, carers and other health professionals, to enable them to make informed decisions regarding treatment and care.
* To comply fully with Infection Prevention and Control Policies.
Aptitudes and Skills

* Excellent interpersonal and communication skills, using a range of methods, e.g. verbal, written, record keeping.
* Strong communication in English, both written and spoken.
* Maintain sensitivity at all times to the emotional needs of the patient and their relatives.

* Able to work autonomously whilst using clinical decision-making skills at an advanced level.

* Computer literacy with a range of software, databases, spread sheets.

* Is able to complete task, two week wait referrals and templates.

Experience
Essential
* Has previous experience working at an advanced level in primary care, urgent care or emergency care.
* Evidence of working at an Advanced level.
* Experience of managing a range of primary/urgent care conditions for adult and paediatric patients.
* Reflect on own practice and direct own learning.
Desirable
* Previously worked as part of an integrated team.
