Cancer Care Navigator Gynaecology

Job overview
An exciting opportunity has arisen for an individual with passion for providing gold standard care for patients who have cancer.
This role will work within a team of Clinical Nurse Specialists (CNS) and lead consultants within the Gynaecology cancer team. If you would like to work in a friendly, passionate and caring team, then this role could be for you.
You will be providing a supportive role to the team, and will be providing administrative and tracking support, working in collaboration with the nursing team to assist patients through their cancer pathway.
Main duties of the job
1. Triage incoming calls, initiate appropriate response according to assessment tools, protocols and individual pathways, liaising with the CNS team as appropriate.
2. Provide basic telephone advice and refer or signpost to other sources of support.
3. Coordinate necessary assessments, appointments, investigations as per the patients care plan, including the use of the Somerset cancer register system.
4. Document and monitor aspects of care coordination and service delivery.
5. Resolve issues which are perceived barriers to patient care.
6. Assist people to access appropriate information and signpost as appropriate.

Qualifications
* NVQ level 3 in business administration or equivalent knowledge gained in practice.
Experience and knowledge/skills
* Experience of coming into contact with cancer patients.
* Experience of working with multi professionals.
* Experience of using hospital IT systems.
* Ability to communicate effectively with patients and professionals.
* Ability to work as part of a team and on own initiative.
* Good understanding of patient centred care.
* Experience of coaching/teaching patients.
