Canvey Island Primary Care Network has a fantastic opportunity for an enthusiastic and committed nurse practitioner to join our team on a full-time basis, for 37.5 hours per week, although this is flexible, to work across all six practices within our network. The main aim of this role is to increase the health and wellbeing of patients through improved screening, identification, and disease management. This will entail a mixture of clinics, ward rounds, outreach events and home visits, as directed. The main responsibilities of the role include: Clinically examine and assess patient needs from a physiological and psychological perspective & plan clinical care. Assess diagnose, plan, implement & evaluate interventions and treatments for patients with complex needs. Proactively identify, diagnose and manage treatment plans for patients with or at risk of developing a long-term condition. Diagnose and manage both acute and chronic conditions, integrating both treatment methods into a management plan. Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national protocols. Implement and participate in vaccination or immunisation programmes for both adults and children. Please see full job description attached. About the Candidate All applicants should have the following qualifications, registrations, skills and experience: A relevant nursing degree. Current NMC registration with an active PIN. Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications. Competence to undertake asthma, COPD and diabetic clinics and reviews, holding relevant certificates. Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care, and recording spirometry. Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice. Knowledge, and experience of its application, in supporting long-term conditions. Insight and understanding of current issues in nursing and the wider NHS. Have strong consultation skills. Be an excellent communicator and collaborator. Have good working knowledge of SystmOne. A non medical, prescribing qualification is desirable. Travel is required between sites and to patients homes.
