To work in partnership with people who are affected by Parkinsons Disease to develop a primary care led Parkinsons Disease service which places the patient at the centre. To provide clinical care to people with Parkinsons Disease. This will include patient and carers advice and support, facilitation and advice on access to therapy and other support services, monitoring and adjustment of medication. The post holder will also be involved in formulation and monitoring of standards of clinical care, promotion of the availability of relevant services for clients and ensure that all clinical practice is evidence based, adhering to national guidelines relating to PD and Parkinsons Plus syndromes including audit of outcomes. To develop and establish entry and exit points on integrated care pathways, self-referral mechanisms for patients to access services and explore methods to improve information, communication and record systems.
