Meadow House Hospice is excited to announce the opportunity to join our well-respected allied health professional team. This service offers the opportunity for staff to be at the frontline of developing and leading on the way we deliver care to our patients, their families, and how we offer support to our partners in caring. We are looking for someone who is passionate about delivering exceptional palliative care, skilled in collaborative working with a variety of healthcare professionals, able to manage complex patient care needs with compassion and efficiency, and committed to improving patient outcomes and supporting our service expansion and development.
Main duties of the job
1. To provide a high-quality and efficient Physiotherapy service to patients with a wide range of physical and medical conditions referred to the Specialist Palliative Care Service.
2. To work as an integral member of the multidisciplinary team at Meadow House Hospice, assisting in the provision of support and rehabilitation to adults, in all End of Care Life settings including the Inpatient Unit, Day Services, and the Community.
3. To provide clinical expertise in the prioritization, assessment, and treatment of patients within the scope of the conditions referred to the Specialist Palliative Care service.

Person Specification
Education and Qualifications
* Degree in Physiotherapy.
* Health Professionals Council Registration.
* Significant post-qualification experience including relevant specialist areas i.e. palliative care, oncology, neurology, or community.
* Membership of a relevant Special Interest Group and/or CSP.
* Evidence of attendance at postgraduate courses relevant to specialist palliative care.
* Evidence of continuing CPD.
Knowledge and Experience
* Experience of clinical audit, clinical governance, and managing risk.
* Experience of managing an independent caseload where patients have life-limiting conditions, and an understanding of specialist palliative care in practice.
* Evidence of staff supervision.
* Accuracy in patient assessment, treatment planning, goal setting, and recording information in all settings.
* Able to participate in end-of-life education.
* Excellent negotiating and leadership skills.
* An understanding of providing services to a diverse population in the community.
Skills, abilities and attributes
* Excellent time management and organisational skills.
* Good problem-solving abilities.
* Ability to work independently and as a member of a multidisciplinary team in all settings of the service.
* Car driver/access to own transport.
