The postholder is expected to work on the middle grade tier in the Urgent Treatment Centre provide the relevant expertise and experience necessary to ensure the efficient primary management of urgent treatment cases. The postholder will be required to work with the other team members in the management of the department workload. The postholder will also be expected to provide supervision to the junior members of the team.
There is a CESR programme in place for those doctors who are interested in pursuing a CESR in Emergency Medicine to gain entry to the Specialist Register. Also offered is the opportunity to undertake quality improvement roles within the department to help gain exposure to management and develop individuals into experienced Speciality Doctors. There is an active programme of multi-disciplinary teaching within the department that successful candidates will be expected to both attend and deliver. We also take part in regular Journal Club meetings with Emergency Department staff from neighbouring Lancashire Teaching Hospitals Trust. Specialty doctors are encouraged to undertake Clinical & Educational Supervisor training and have the opportunity to formally supervise junior doctors and medical students from UCLAN and Lancaster Universities. A Consultant mentor will be provided as support for individual development.
Specialty Doctor in Urgent Treatment Centre
Established in 2003 ELHT is a large integrated health care organisation providing high quality acute secondary healthcare for the people of East Lancashire and Blackburn with Darwen.
Our vision is to be widely recognised for providing safe, personal and effective care. We currently provide high quality services and treat over 700,000 patients a year from the most serious of emergencies to planned operations and procedures. We employ over 8,000 staff, many of whom are internationally renowned and have won awards for their work and achievements.
The department itself has a large multi-disciplinary team encompassing Consultants, Specialty Doctors, Higher Emergency Medicine Trainees, Junior Clinical Fellows, Foundation doctors, General Practice Trainees, Advanced Care Practitioners, Physicians Associates, Emergency Nurse Practitioners and Physiotherapy practitioners. As well as the nursing team. The department also supports the education of medical students from UCLAN and Lancaster Universities. We are a teaching unit providing teaching for all grades of staff.
You will be assigned an educational supervisor who will meet with you regularly to ensure your educational needs are met whilst you are with us and we encourage Junior Clinical Fellows to undertake the Life Support Courses whilst you are with us. To aid this you will have access to a study leave budget. There is simulation training which is run on the shop floor and that you will be encouraged to attend.
Job Summary
Specialty Doctor in Urgent Treatment
For entry to the Speciality Doctor grade, applicants must have at least four years of post-registrationtraining, of which at least two years must be in a speciality training programme in Emergency Medicine (for
example, ACCS) or must be able to demonstrate comparable skills and competencies for example having completed General Practitioner Training. The job title of the post is Speciality Doctor in Urgent Treatment Medicine. The post is paid on the Speciality Doctor scale. Applicants wishing to work part-time would be considered.
Job Plans are specific to the post and generally 10 PA posts with a split of 8.5 DCC and 1.5 SPA. The department has experience of supporting doctors in CESR (previously Article 14) applications, and
again this would be encouraged for suitable experienced doctors.
Duties of the Post
The postholder is expected to work on the middle grade tier in the Urgent Treatment Centre provide the relevant expertise and experience necessary to ensure the efficient primary management of urgent
treatment cases. The postholder will be required to work with the other team members in the management of the department workload. The postholder will also be expected to provide supervision to the junior
members of the team. The postholder will be expected to undertake full clinical duties including the running of clinics or assessing
and managing acutely unwell patients, along with patients presenting with injuries both major and minor. The care may very occasionally include initial resuscitation where appropriate. The postholder will be
expected to manage the caseload within urgent treatment and organise immediate management and plan for discharge or where required referral for follow up. The referral could be to GPs to an appropriate
specialist team for continued hospital care or to other services.
The postholder will be expected to follow the various guidelines and protocols issued in the department. In the case of major trauma cases, cardiac arrests and critically ill patients attending the department, the
postholder will be expected to act in a team leadership role, following appropriate life support guidelines.
Postholders would be expected to gain and maintain certification in ALS, ATLS etc and APLS or recognised equivalents to these courses.
In light of the changing work practices, the work programme may need to be varied in the future. This will be done only after consultation with the incumbent of the post. As duty hours require the incumbent being
present in the department, no on-call or residential accommodation is offered with this post. In exceptional circumstances, additional duties outside normal rostered hours may be required to ensure
the continuation of essential patient care. So far as is consistent with the proper discharge of the above duties, the postholder may be required to deputise from time to time for absent colleagues. A mentoring process will be in place to make sure that the professional and personal development of the postholder are maintained. The Consultants will act as mentors and will meet with the postholder on a
regular basis.
Teaching, Audit & Research
The postholder will be expected to participate in teaching and training of junior doctors, medical students, nurses and paramedical staff as directed by the Clinical Lead. The postholder will also be expected to participate in the educational programme and attend relevant postgraduate courses in accordance with the local and national CME requirements.
The postholder will also be expected to participate in the departmental audit programme. Clinical research will be encouraged.
Emergencies and Unforeseen Circumstances
The postholder accepts that they will also perform duties in occasional emergencies and unforeseen circumstances, without additional remuneration at the request of the Clinical Lead or the on-call Consultant. It has been agreed between the professions and the Department that while the postholder accepts that they
will perform such duties, it is stressed that additional commitments arising under this heading are exceptional and the postholder should not be required to undertake work of this kind for prolonged period or
on a regular basis. It is the responsibility of the Clinical Lead to determine whether a particular situation can be described as
an emergency.
Study and Training
Postgraduate study is encouraged. In-service training is available and clinical meetings are held in the Department. There is an excellent postgraduate medical centre at Royal Blackburn Hospital which
provides excellent library facilities and an active teaching programme, including guest speakers, clinical conferences and departmental tutorials. Study leave is awarded at the discretion of the Clinical Lead.
The Department
The Emergency Department is located at the Royal Blackburn Hospital a newly built PFI hospital with fully integrated Paediatric and adult areas with Resus, PACS system, and fully supported by CCU and ITU
Assessments Units, X-ray, CT and MRI facilities. There is a hospital trauma team and cardiac arrest is managed in house.
In additional to the main Emergency Department which takes primarily ambulance cases, there are 2 Urgent Treatment Centres catering to minor injuries and illnesses assessing both walk-in and appointed
patients. One is situated in Burnley General Hospital and other co-located within the Emergency
Department at Royal Blackburn Hospital.
The Emergency Department has an 8 bedded resuscitation area (with 1 bed designated for paediatric patients) with 42 major cubicles. The two Urgent Treatment Centres have examination cubicles, staff base,
separate adult and children waiting areas, reception and all facilities to see minor injury and illness. The department receives excellent support from the Intensive Care Unit, Medical Assessment Unit and Surgical
Assessment Unit, Paediatrics, Orthopaedics, ENT and Maxillofacial Surgery. Serious burns and neurosurgical patients are transferred to the regional centre at Preston or Manchester. The Departments have annual attendances of approximately 170,000 across the three sites. The Acute Medical Unit has been established both for medical patients referred from the Emergency Department and Urgent Treatment Centre and for direct GP referrals. Patients who are referred by their GP to other specialities are ordinarily directed straight to the appropriate ward or assessment area. There are ambulatory medical and surgical day emergency care units.
There are currently 18 Consultants and two Specialists who work on the Consultant Rota in post in the unit.

Burnley UTC is lead by two Consultant General Practitioners, Dr Zulfikar Aslam and Dr Raju Uppalapati.
Other Clinicians include 4 higher specialist trainees, 28 speciality doctors, 57 junior doctors made up of ACCS & GP trainees, junior clinical fellow and foundation year 1 and 2 doctors. We also have Advanced
Care Practitioners, Emergency Nurse Practitioners, Physicians Associates and First Contact Physiotherapists. We train staff of all grades and have medical students of various years from UCLAN and
Lancaster University in the department.
Terms and Conditions of Service
This post is subject to the Terms and Conditions of Service for Speciality Doctors – England 2021. The appointee will be required to live within 10 miles or 30 minutes travel time by car from the Royal Blackburn Hospital. The Trust will reimburse certain expenses relating to removal and relocation when required, the expenses refunded shall reflect actual costs involved but will not exceed £8,000. If relocation
is required this must be discussed as interview. A satisfactory medical examination is a condition of employment for medical and dental staff in the NHS.
