Main Duties Of Job Contacting patients by telephone, email, text message, or letter to make appointments for healthcare professionals as required for long term condition management. Supporting triage of information received via AccuRx or other platforms. Providing administration support for the practice team. Using internal and external email and the internet to communicate and keep up to date. Supporting patients with referrals to external services such as Social Prescribing and befriending services. Responding to all queries and requests for assistance from staff and other visitors. Technical and Administrative Using SystmOne to access patient records, book blood tests, and arrange routine appointments. Helping people manage their needs through answering queries and providing quality information. Providing expertise to address public health and social care needs, including lifestyle advice and service information. Ensuring appropriate onward referral of urgent issues to clinicians. Supporting the coordination and delivery of multidisciplinary teams. Maintaining records of referrals and interventions for monitoring and evaluation. Using appropriate SNOMED codes to record activity. Enable Access to Personalised Care and Support Having positive conversations with patients, families, and carers about their needs. Working to increase patients' understanding of health and wellbeing management. Collaborating with the wider PCN and MDTs to support personalised care. Assisting patients to manage health inequalities. Coordinate and Integrate Care Making and managing appointments related to various organizations for patients. Helping people transition between secondary and community care services. Referring to social prescribing link workers and health and wellbeing coaches. Liaising with multidisciplinary professionals to ensure coordinated care. Participating in multidisciplinary team meetings as appropriate. Recording interventions and progress in health and care journeys. Identifying service gaps and providing feedback for improvement. Work in accordance with practices and PCNs policies and contribute to improving primary care.
