The following are the core responsibilities. There may be, on occasion, a requirement to carry out other tasks; this will depend on factors such as workload and staffing levels: Undertake responsibilities in line with practice demand for childhood vaccinations Provide education and advocacy about the benefits and safety of vaccinations, addressing any concerns in misconceptions individuals and their carers Work with practices to facilitate easy access to immunisation services Track immunisation rates and follow up with families. Build and maintain relationships with patients and families with consideration to health inequalities and diversity Participate in outreach activities to raise childhood vaccination awareness and uptake Work closely with other health, social and educational care professionals, such as local schools, childcare providers, to identify and support patients making sure that access to appropriate vaccinations is available to them and ensuring that their needs are met Report on progress, challenges and outcomes to management and stakeholders Proactively identify and work with registered patients, their families and carers to support their personalised care needs in order to achieve vaccination uptake Ensure that people have good quality information to help them make educated choices to vaccinate. Signpost support to patients/family/carers on information about immunisations Maintain accurate and contemporaneous patient records at all times Demonstrate flexible attitude and be prepared to carry out other duties as may be reasonably required from time to time within the general character of the post or the level of responsibility of the role, ensuring that work is delivered in a timely and effective manner.
