Main duties of the job As a Clinical Reviewer your main duties will include: Clinical Expert and Chair of Care and treatment Reviews (CTRs) Writing the KLOE report for CTRs Ensuring the persons views are heard during a care and treatment review as well as assessing the clinical needs of patients and people within the community who have a learning disability and/or autism. Leading conversations within multi-disciplinary teams during CTRs In discussion with the CTR panel devise SMART objectives for the person and their team LeDeR reviews As a Leder Review you will use your professional curiosity to find information about a persons life and death Analyse information against standards for good care and treatment Write reports highlighting effective practice and areas for improvement You will communicate with other professionals, families and providers to gather information. You will present reports to local ICB quality assurance panels Independent Life Plans You will build a relationship with the person with ASD and/or LD and gather information to write a life plan. You will liaise with the persons circle of support to gather clinical and social needs You will analyse information in order to develop recommendations You will present information in a written format which is professional yet person centred. You will talk to families about the persons history and needs. You will also need to provide reports and updates on a regular basis to outside agencies and your line manager.
