The current postholder is retiring and therefore this post would not be available until 27th December, 2024 Main duties of the job You will expected to support holistic and clinical assessments in order to meet the patients needs, providing education regarding how to manage their long term condition whilst also working alongside a team of other professionals (District Nurses, Community Therapists, Integrated Care Coordinators, Health and Wellbeing support Workers etc within Fylde Enhanced Primary Care Team). The role will include building good relationships and working closely with GP's to ensure seamless, effective and timely care.
