Care Coordinator - Early Intervention Team

The Early Intervention Team works with people ages 14 - 65 experiencing their First Episode of Psychosis.
To participate in the provision of high quality care. This will involve assessing, supporting, monitoring of the service users who are experiencing First Episode Psychosis and those who are suspected of developing psychosis. It will involve working in a variety of community settings; it will include liaising and co-working with other services both statutory and non-statutory. It will involve working with carers and families of service users and will involve seeing service users' families and carers in their own homes, clinics or in other environments such as hostels, supported living and residential settings.

* Have full driving license and use of a car or access to a car to be able to take passengers/clients in car

Education / Qualifications
* R.M.N. / RNMH/ DipSW / SROT
Experience
* Experience working with clients with serious mental illness within an Early Intervention Psychosis Team
* Experience of teamwork and effective communication at all levels
* Ability to work independently in a community setting
* Experience in the use of psychosis-specific semi-structured interviews (e.g. PANSS; CAARMS; PSYRATS)
Knowledge
* Knowledge and skill in assessment, planning, implementation and evaluation of the care of clients with serious mental illness within an Early Intervention in Psychosis team under the supervision of a senior clinical practitioner
* Knowledge of legislation that underpins the care of those with serious mental illness
* Knowledge of NICE guidance applicable to those with psychosis and schizophrenia
Skills and Abilities
* Able to work within workload deadlines and prioritise workload
* To organise, plan and implement own work, under the supervision of a senior clinical practitioner
Work Related Circumstances
* Able to demonstrate flexibility in working hours required to meet client needs
* Use of a car or access to a means of mobility to travel across the Trust footprint in line with service needs

What happens after your application has been received?

What happens if I am offered the position after interview?
