Job overview
The post holder will lead in the review of BSWICB ABI care packages, following discharge from Neurorehabilitation placement, identifying outcomes for the assessed needs of individuals and assisting in finding specialist placements for individuals with non-CHC acquired brain injury.
Main duties of the job
Coordinating and undertaking placement reviews and assessments and completion of CHC checklist for individuals with ABI. This necessitates a high level of assessment skills requiring tact, diplomacy and the ability to liaise effectively with colleagues, health care and multi-disciplinary professionals, service users, carers and family members.
The timely review of all service users to ensure monitoring of quality assurance, ensuring the appropriate level of care and funding, and the accuracy of data held by the Non-CHC ABI under the All Age Continuing Care Commissioning Team.

Detailed job description and main responsibilities
· Support with specific projects and or reports, as required.
· Update, maintain, organise, gather and analyse information to predict and or meet future organisational and team needs.
· In relation to placements: monitor and track risks and issues establishing a proactive resolution and escalation processes.
· Coordinate and participate in relevant meetings, reporting attendance and providing information, advice and support where requested.
Ensure that data collected, in relation to ABI placement assessments and reviews, is analysed and reported by the team as appropriate.

Qualifications
* Clinically Qualified (e.g., RMN/RNMH/RGN/RNLD)
* Evidence of continuing Professional development
* Experience in managing and supporting complex care packages is essential.
* Experience for further study in Acquired Brain Injuries (Korsakoff and Functional Neurological Disorder)
Knowledge
* Experience with NHS or Local Authority.
* Knowledge of continuing care policy and procedures.
* Minimum of three years experience in relevant clinical practice.
* Knowledge of MHA, Consent to treatment, DoLs.
* Knowledge of Social Services eligibility systems and procedures.
* Experience of interpretation of national policy and implementation.
* Understanding of the wider health and social care agenda.
* Flexible and able to work as a member of the team.
* Excellent planning and organisational skills.
Abilities
* Ability to assist in the development user care developments.
* Ability to exercise judgement from professional and or clinical assessments and make decisions.
* Ability to present information in various formats to differing audiences.
* Ability to communicate effectively across the health, social and independent care community.
Communication Skills
* Effective communication skills; verbal, written and non-verbal.
