Job summary Previous coding experience highly desirable. To review all incoming correspondence and enter, using SNOMED codes, pertinent information into the patients electronic medical record in accordance with the coding protocol. This will include data highlighted by clinicians as well as data required to meet contractual obligations. To organise the medical record of patients whom you are coding, to group problem titles, group symptom development and assign the appropriate status to medical information. To recognise own knowledge limits and seek advice from clinical or administrative colleagues as appropriate. Main duties of the job a) Read all incoming correspondence to the practice, accurately coding the relevant diagnosis / procedure / information in the patients records, deciding whether to send on to a GP, ensuring follow-up actions / tests are brought to the attention of the relevant staff members (GP, pharmacists, admin). b) Use SNOMED coding into Emis to ensure the information is attached to the relevant correspondence. c) Sort and date stamp all incoming patient-related mail. d) Scan patient-related documents onto their medical record using filing codes as agreed by the clinical team. e) Ensure incoming letters are scanned onto the patient record within a 48/72-hour timescale. f) To email blood test, or other relevant information to outside agencies and Secondary care to ensure the continuity and safe care of the patients. h) To move the relevant lab reports, prescriptions and documents to the appropriate doctors to ensure that no results are missed due to annual leave etc. i) To deal with Out of Hours reports, BCS FOB results, Cytology and GP2GP records as appropriate. j) To manage admin emails, action and complete forward as appropriate k) To summarise and read code relevant information from patients clinical records into the practice computer system, working strictly in accordance with the Note Summarising Protocol.
