Main duties of the job
The successful post holder will be expected to develop and deliver pharmacy services to patients at Medway Hospital.
As the Rotational Senior Pharmacist, you will ensure that patients get the most from their medicine and receive optimal support.
The post is rotational within Pharmacy. Clinical rotations include (but are not limited to) Acute medicine, Admissions, Cardiology, Surgery, Paediatrics, and Critical Care (level 2 and level 3).
In addition, the post holder will gain experience in taking on additional areas of responsibility including: Anticoagulation, Aseptic services, Governance and Risk, Research and audit, and Antimicrobials.
The post holder will liaise between wards, departments, and pharmacy to ensure that pharmacy services are tailored to meet the Trust's and patients' needs.

Job description
Job responsibilities
To provide a pharmacy service to the wards and departments within Trust whilst working alongside other pharmacists, technicians, and assistants.
Antimicrobial Stewardship
* Work with Lead Antimicrobial Pharmacist to review antimicrobial policies and protocols to ensure they reflect current and evidence-based practice, and are governed by local resistance information.
* Ensure antibiotic guidelines, policies, and strategies are adhered to.
* Assist the Lead Antimicrobial Pharmacist in managing, maintaining, and updating the Antimicrobial Prescribing application.
* Provide proactive input into the antimicrobial management of patients with infections/sepsis and inappropriate antimicrobial use.
* Liaise closely with the Infection Management Team and attend meetings to discuss anti-infective issues and ensure adherence to measures to reduce Healthcare acquired infections.
* Undertake audits to monitor adherence to antimicrobial policies, providing feedback to divisions and individual prescribers where appropriate.
* Provide antimicrobial usage and supply information via Defined Daily Dose data and feedback to the Antibiotic Stewardship Group.
* Be part of the Antimicrobial stewardship committee, actively promoting stewardship across the Trust.
* Act as an antimicrobial resource for all Clinical Staff within the Trust, particularly on treatment regimens, surgical prophylaxis, defined durations of therapy, and guidance on IV to oral switch and TDM.
* Engage with the Hospital at Home (H@H) and Surgical Patient Enhanced (SPET) nursing teams ensuring that outpatient antimicrobials and assays are safely managed.
* Attend and actively contribute to consultant-led antimicrobial ward rounds.
Patient Services
* Dispense and check dispensed medication as required.
* Provide a link between clinical and patient services, supervising clinical pharmacy practice within the dispensary as required.
* Ensure adherence to prescribing policies pertinent to patient services, including the use and supply of unlicensed drugs and licensed drugs used outside license.
* Support the Dispensary Management team to ensure that tasks are carried out in accordance with departmental procedures and protocols by all staff in the dispensary areas and to record and implement measures to reduce dispensing errors.
Education and Training
* Be a Practice Supervisor (PS) or an Educational Supervisor (ES) for Foundation Pharmacists undertaking the postgraduate Diploma in General Pharmacy Practice.
* Be a Practice Supervisor for Pre-Registration Pharmacists and, if appropriate, to be an Educational Supervisor.
* Undertake Supervised Learning Events (SLEs) and ward assessments with Foundation Pharmacists.
* Participate in the education and training of pharmacy staff, other healthcare professionals, and pharmacy students to graduate level.
* Supervise the day-to-day working of Foundation Pharmacists and Pre-Registration Pharmacists.
* Maintain an active portfolio of continuing professional development (CPD).
* Participate in departmental CPD educational sessions.
Policy Development, Financial & Resource Management
* Update or write appropriate standard operating procedures.
* Support the development and implementation of policies to control drug expenditure and ensure cost-effective prescribing.
* Support the Lead Pharmacist Clinical Services and Principal Clinical Pharmacists in monitoring drug expenditure and providing accurate expenditure information, with appropriate analysis, to the Division.
* Identify areas of potential cost-savings, and implement systems to realize savings and contain prescribing costs.
* Educate other healthcare professionals on the costs associated with drug therapy and promote the use of evidence-based, cost-effective prescribing within the division.
* Assist the Lead Pharmacist Clinical Services and Principal Clinical Pharmacists to implement local and national directives, policies, and procedures e.g., NICE, Patient Safety Alerts.
Audit and Research
* Undertake clinical audit and research work within Pharmacy and in conjunction with other healthcare professionals.
* Contribute to the implementation of change arising from audit projects and Quality Improvement projects.
* Publish and present the results of such work locally, nationally, and internationally.
* Supervise audit and other quality improvement projects undertaken by pharmacy students, pre-registration pharmacists, and pharmacy technicians.
General
* Report any complaints, incidents, or near miss situations via the error reporting system and your line manager.
* Maintain an active portfolio of continuing professional development (CPD).
* All pharmacy staff must show confidentiality regarding any information about patients, confidential information about the Trust, and commercial information (e.g., drug prices).
* Uphold the principles of Clinical Governance within the Trust.
* All work will be carried out in accordance with the relevant laws, the guidelines of the Royal Pharmaceutical Society of Great Britain and the General Pharmaceutical Council, Trust and departmental procedures, and standards of professional practice.
* Undertake any other relevant duties as may be required by the Chief Pharmacist.

Qualifications
Essential
* Master's degree in Pharmacy and registered with GPhC as a Pharmacist in the UK (or eligible to be).
* Diploma fully complete (1 point) only partly complete or certificate only (1 point).
Experience
Essential
* 18 months of post-qualification experience within pharmacy.
* Hospital pharmacy experience.
* Evidence of MDT working.
Knowledge
Essential
* Evidence of good core therapeutic knowledge.
* Evidence of understanding of medicines management.
* Demonstrates an understanding of medicines safety.

* Evidence of excellent communication skills (verbal and written) with healthcare professionals and patients.
