Hours: 37.5 per week
Channel Health Alliance has exciting new opportunities for a Paramedic or Nurse wishing to join primary care services. The organisation works closely with the East Kent Training Hub to ensure staff have access to educational opportunities for professional development.
We are looking for enthusiastic individuals with advanced skills in patient care delivery, including:
* Holistic, evidence-based patient care
* Illness assessment
* Diagnosis
* Complete home visits as part of our home visiting team
Main Duties of the Job
The post holder will have a special interest in primary care and acute assessment of patients within their own homes, liaising with colleagues from the multidisciplinary team and GP practices to support the patient at home. The service delivers a 72-hour wrap-around service to reduce hospital admissions.
We seek individuals who wish to make a difference in the care and treatment of patients and can embrace the 5-year forward view and NHS Long-term plan to ensure the changes are delivered within both primary and urgent care.

Job Responsibilities
To practice as part of Channel Health services working as a paramedic or nurse within the home visiting service. The role involves:
* Completing home visits for primary care practices across the south Kent coast.
* Providing on-the-day assessment and up to 72 hours follow-up for patients to prevent hospital admission.
* Providing care for patients with a variety of illnesses, completing clinical assessments and formulating diagnoses discussed with the GP to create a treatment plan.
* Working collaboratively within the team and completing referrals to relevant organisations to support patient care.
Clinical Responsibilities
* Provide and maintain a high standard of clinical assessment and treatment.
* Support GPs and other members of the Health and Social Care services.
* Deliver care within the boundaries of their role and competence, using advanced clinical skills to educate patients and promote self-care.
Person Specification
Qualifications Essential
* Professional Registration/ registered clinician & registered with regulatory body
* Good communication skills and educated to GCSE grade A-C in English and Maths.
* Basic IT skills
Experience Essential
* Ability to manage and prioritise workload.
* Proven ability to work autonomously and as part of a team.
* Current knowledge of government targets, initiatives, and NICE guidelines.
* Understanding of professional Code of Conduct.
* Knowledge of adult and child protection issues.
* Good communication skills.
* Experience of triage/initial assessment.
