Due to the retirement of a Diabetes Specialist Nurse, an exciting opportunity has arisen for a motivated and dynamic Diabetes Specialist Nurse to join the community team at Herts Valleys Integrated Diabetes Service (HIDS) in South and West Hertfordshire. The role includes clinics (face to face or telephone) and group education sessions.
The role is for 16 hours a week and there is flexibility in your work pattern.
The successful candidate will work as part of the wider HID Service, which is an integrated service working across community and acute settings.
Hertfordshire Community NHS Trust works very closely with our partner organisations - West Herts Teaching Hospital Trust, Hertfordshire Partnership University Foundation Trust to provide an integrated service to adults with diabetes in South and West Hertfordshire.
In addition to the basic salary, this post attracts a High Cost Area Supplement of 5% of basic salary subject to a payment of £2,122 per annum (pro rata where applicable).
Main duties of the job
We are committed to delivering a quality service, value staff training and CPD, participate regularly in student training and have a robust appraisal and supervisory framework. Electronic records are used throughout the Trust, further aiding our ability to work across a wide variety of settings and be in a position to work as part of an MDT, yet work remotely if needed as part of a hybrid approach to our work/life balance.
