Responsibilities:
* Conduct comprehensive assessments to determine individuals’ needs in accordance with the principles of the Social Services and Wellbeing Act.
* Develop and implement care plans in collaboration with multidisciplinary teams.
* Facilitate access to community resources.
* Conduct capacity assessments and address best interests.
* Advocate for individuals’ rights and services, ensuring ethical and legal compliance.
What we are looking for in a Social Worker:
* Social Care Wales registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
