This post is situated within the NHS Sussex Team. Staff will support each other in service delivery across Sussex for all children and young people registered with a Sussex GP. This will require the post holder to work across the Sussex wide county in order to meet the needs of the service. The Nurse Assessor is responsible for: Supporting the commissioning of health care packages at home for CYPCC eligible children and other community settings to prevent hospital admissions; in addition, they will help support early discharge from hospital where ever possible, working with the Well Child nurse for Pan Sussex. In the absence of Nurse Specialists, act in a deputising role, which would include undertaking duty management and responding to package breakdowns, enquiries for new referrals, checklists and prioritising workload. To help support the daily duty system for CYPCC across Pan Sussex, working with the duty nurses; responding in a timely manner to support the Pan Sussex caseload. Acting as a member of the multidisciplinary team to co-ordinate the process, establish eligibility and assist in identifying a suitable care package or respite setting. The post holder will be responsible to ensure that the review process for all commissioned care is undertaken in accordance with current guidance. Assessing children and young people in the community/ home and hospitals to determine eligibility to receive NHS Childrens Continuing Care. Utilising the national assessment tools and adhere to the required timescales. Co-ordinate, participate and support the assessment process with the multidisciplinary team (MDT). The role will be part of the MDT and determine the individuals eligibility in line with the National Framework for Children and Young People's Continuing Care. This will encompass explaining the process to patients and families, health and social care colleagues and care providers. Ensuring that the process is transparent and clearly understood by all parties. Clinical leads for CYPCC will support throughout this process. Assist when required with the monitoring and providing assurance on the quality of service providers for Children and Young Peoples Continuing Care, this includes residential placements and short breaks environments. Advising CHC Procurement on clinical needs and best practice in relation to services for children with complex health needs and providing clinical input to all parts of the commissioning process. Working alongside the NHS Sussex PHB team, assist with the personal health budget (PHB) process for childrens families / carers who would like to be in receipt of a personal health budget and ensure that the NHS Sussexs procedures are monitored alongside the PHB navigator. To be responsible for planning, managing, co-ordinating and undertaking annual reviews of the Children and young peoples eligible for Continuing HealthCare, within the Pan Sussex caseload. Responding to any health needs reported and reviewing the DST and care package accordingly.
