This is an exciting opportunity to work within a busy community nursing team as a band 6 Case Load Manager.
The Team is well established with experienced case load managers who will be eager to support and mentor if you are moving up from a band 5 post.
You will be responsible for the administrative and face-to-face care coordination of one or more GP Practices and the support, development and wellbeing of the junior staff assigned to them.
When coordinating, you will allocate the team's patient visits, deal with referred tasks, support junior staff, and stand in for the Team Leader when they are off duty.

Clinical Responsibilities
1. Act as an autonomous practitioner in accordance with and upholding the NMC Code of Conduct and Scope of Professional Practice for Nurses, Midwives and Health Visitors.
2. To manage a caseload of patients.
3. To assess physical and psycho-social needs, plan, manage, implement and evaluate individual programmes of patient care in the most efficient and clinically effective way, which meets the needs of the patient and his/her carer(s).
4. To improve health outcomes using specialist nursing skills and knowledge.
5. Carry out complex nursing care and procedures.
6. Use a range of assessment tools pertinent to the patient’s needs to inform the assessment and assess risk for both patients and staff.
7. Work unsupervised to assess, plan, implement and evaluate clinical care of patients. Provide skilled nursing care for patients in their own homes or other appropriate settings e.g., clinics.
8. Ability to plan ahead for potential scenarios to ensure anticipatory care needs are understood and met.

To have overall responsibility for managing planned programmes of care for patients in domiciliary and clinic settings. To assess, plan, implement and evaluate nursing care to patients across the community/primary care interface in the most efficient and clinically effective way, in co-operation with other members of the health and social care teams, and voluntary and statutory agencies.
To work with the team leader to allocate appropriate workloads to team members; both registered and non-registered staff. The post holder will work with a range of professionals in order to ensure care is appropriate and delivered in a timely manner. The post holder will be able to demonstrate their professional skills and competence in leadership, care planning/co-ordination, medicines management, and care and treatment delivery.
To manage workers and resources efficiently and effectively under his/her control, to provide the highest possible standard of care for patients. To provide advice and support to junior staff in the delivery of high-quality care and actively support the team leader in the management of the team.
