The Cancer Services department at Frimley Park Hospital is a busy service comprised of Tumour Site Specific Multi-Disciplinary Teams.
We are looking for a highly motivated and proactive individual, who has experience of working within a team environment, who will virtually navigate patients through their care pathway from point of referral to treatment in accordance with agreed pathway steps and timescales, proactively identifying and resolving delays at all stages. They need a strong understanding of the two week wait, 28 day, 31 day and 62 day targets, will ideally have previous NHS experience, along with excellent IT, communication, interpersonal skills and the ability to prioritise and organise their own work with minimal supervision.
The successful applicant will be a person that embodies the Trust Values and has the competencies to fulfil the core role, which requires the ability to efficiently and effectively co-ordinate tumour site specific Multi – Disciplinary Team meetings, track patients against Cancer Waiting Times targets to ensure Minimum Data Sets and Audits are completed, and capturing and accurately recording the relevant data for each patient in line with the National Data requirements.

• To actively track patient’s progress through the patient pathway to ensure that care is carried out in a timely fashion on daily basis and follows escalation and tracking process on PTL (Patient Tracking Tool).
• Specific co-ordination and actioning of process of cross site referrals to another specialty (internal within the Trust)
• General co-ordination and actioning of MDT – eg. Tracking patients post MDT, updating with requested investigations, referrals to tertiary centres, and general pathway progress on SCR
• To facilitate multidisciplinary team meeting discussion and ensure cancer data is captured and recorded on Somerset
• To ensure that all cancer patients are identified and discussed at MDT meetings.
• To ensure patients’ diagnoses, investigations, and management and treatment plans are recorded on Somerset to ensure cancer data uploaded is accurate.
