We’re looking for a Social Worker to join our Discharge to Assess Team (D2A), supporting the timely assessment of individuals who have been discharged following the D2A process. You will be the aligned Social Worker assessing people’s need for care and support under the Care Act 2014, using a strength-based approach and arranging person-centred support that promotes independence.
We take a living well approach in Norfolk which means supporting people in the context of their own families, communities and circle of support; empowering people to remain independent for longer.
