Job Overview
Brain Injury Case Managers are responsible for working with clients with brain injuries (and other neurological impairments) to coordinate and implement rehabilitation, care and support plans, advocate for client needs, and provide support to both the client and their family.
As a Case Manager, you will be responsible for providing support and guidance to individuals requiring assistance in navigating social care, therapy and healthcare systems. You will work closely with clients, families and colleagues to assess need and to ensure our clients receive optimum support.
Duties
1. Conduct assessments to determine clients' needs and goals
2. Develop individualised Case Management and care and support plans in collaboration with clients, family members and colleagues
3. Coordinate and monitor services provided to clients
4. Advocate for clients' rights and access to services
5. Write reports and provide estimated costs for services
6. Maintain accurate and up-to-date case records
7. Provide mentoring and supervision to colleagues
8. Solve complex problems
Requirements
1. Proven experience in Case Management or a related field
2. Experience working with people with brain injuries or in a related field such as rehabilitation and neurological disabilities
3. Excellent communication and interpersonal skills
4. Ability to work independently and as part of a team
5. Knowledge of social care services, therapy provision and healthcare systems
6. Experience in mentoring and supervising
