Role Overview: First point of contact for patients and service users. Management of team email address for referrals and queries.
On receipt of all new referral notifications, ensure that the validation process has been followed in line with departmental guidelines, liaising with GP surgeries, hospital trust, or other stakeholders. Responsible for appointment booking in a timely manner.
Key Responsibilities:
1. Manage and book clinics in relation to service growth, communicating with venues and building a clear schedule in line with staffing capabilities.
2. Ensure accuracy of patient demographic details, making amendments/corrections in a timely manner.
3. Input all data accurately into the bespoke screening database, or RIO as appropriate, in a timely manner.
4. Maintain a clear and up-to-date knowledge of the rules and protocols of the service.
5. Regularly validate all extracted diabetic reports from GP surgeries across Staffordshire.
6. Ensure all patient and GP correspondence is sent within required timescales.
7. Dispatch patient satisfaction surveys and input returns in line with Trust requirements.
8. Make, amend, or cancel appointments as required by service or patient.
9. Monitor and order office supplies/stationery and clinical equipment/consumables, ensuring their safe storage/retrieval.
10. Send specimen jars to patients in time for clinical procedures.
11. Coordinate appointments between diabetic eye screening and the eight care processes.
12. Update patient records.
13. Undertake any other duties and responsibilities as required consistent with the grade.
Additional Information: Car user with a full driving licence will be beneficial due to bases covered.
