Band 3 Integrated Support Worker - Community
The Urgent Community Response team are looking to expand the team of Integrated Support Workers across East Sussex with opportunities to be based in either Eastbourne or Hastings.

The Urgent Community Response team supports patients within their own homes across the UCR, HomeFirst and Virtual Ward pathways. Main duties of the job
* Support people to wash and dress promoting independence.
* Encourage and motivate patients with managing their nutrition.
* Medicines management from an administration chart under the supervision of an Integrated Senior Support Worker.
* The crisis teamwork from 8am-10pm 7 days a week, shifts are flexible with full and part time opportunities.
* Independent travel between patient's homes and the office base in a timely manner.
The ISW role is an opportunity to provide outstanding care and support to patients in their own homes. The role involves working alongside Senior ISWs, Nurses and Therapists to optimise patient outcomes. ISWs provide personal care, meds support and support with eating and drinking as well as working towards further competencies to further support the Multidisciplinary team.
The Urgent Community Response team work alongside the integrated night service who provide cover overnight.

Person Specification
Qualifications / Training
* Good standard of general education, including English and Maths.
* National Vocational Qualification (NVQ) Level 3 or equivalent experience.
* Satisfactory General Education and evidence of additional training relevant to role.
* Educated to GCSE level.
Other
* Ability to fulfil the travelling requirements of the post.
Experience
* Previous experience in nursing or social care setting.
* Experience of working independently as well as part of a team.
* Previous community nursing and/or Physiotherapy, Occupational Therapy experience.
* Experience of working in a community setting.

Salary: £22,816 to £24,336 a year, pro rata.
