We have an exciting opportunity for your growth within our new Single Aged Care Assessment team. Our goal is to provide a cohesive and efficient approach to caring for the elderly, ensuring comprehensive support for our clients.

Responsibilities:
* Manage team Aged Care Needs Assessments Key Performance Indicators as contractually directed.
* Lead the Assessor team to deliver high quality in-person and remote comprehensive assessments.
* Develop and monitor strategies for assessors to connect people with appropriate services and supports in their community to further empower the client with their independence.
* Engage in peer review processes with triage and assessment delegates and assessors.
* Assign, un-assign, and reassign referrals for triage and assessments.
* Provide ongoing support and mentorship to assessors in the field.
Essential Requirements:
* Tertiary qualifications in a health-related discipline (e.g., Medical Officer, Registered Nurse, Social Worker, Occupational Therapist, Physiotherapist, or other Health Professional).
* At least two years of experience in a leadership or supervisory role in aged care or a related field.
* Sound knowledge of Home Care Packages, Short term Restorative Care Program, Transition Care Program and Residential Respite Services.
* Previous experience performing in a KPI environment to ensure quality client outcomes.
* At least one year of experience in a related role is preferred.
* Unrestricted Australian Drivers Licence.
* National Police Check.
