Job summary A Therapy Patient Flow Co-ordinator role has arisen for an individual with strong communication and problem-solving skills who is passionate about enhancing patient experience. In this role, you will be responsible for facilitating the timely, effective, and appropriate discharge of Therapy patients from the acute setting. You will work closely with Therapy clinicians, patients, families, and members of the multidisciplinary team across hospital, community, social, and health services to help reduce delays in discharge. The successful candidate will also be involved in identifying and leading initiatives to support patient flow throughout the hospital, as well as coordinating and planning complex discharges. The interview will be held on the 3rd and 4th of April. Main duties of the job Act as the main contact point within Therapies for escalations regarding discharge planning from clinical and non-clinical MDT and Health and Social care partners. Communicate highly complex patient-related information effectively to ensure collaborative working with multidisciplinary team members across health and social care sectors, ensuring the delivery of a co-ordinated multidisciplinary service. Develop and implement specialist discharge care plans. Identify, recommend, and implement service improvements within discharge processes and patient flow. Use professional judgement when complex non-routine issues arise, assessing situations fully and taking appropriate course of actions and escalations for best patient and Therapy service outcome.
