JOB SUMMARY: Responsible for delivering skilled nursing care to patients in home and community-based settings. To be responsible for the assessment, coordination, implementing and evaluation of care for Neighbourhood Rehabilitation patients, reporting and escalating relevant information to appropriate health and social care professionals. Deliver evidence-based care, which is safe, effective, in a manner which is caring, compassionate and enables individuals to reach an optimum level of functioning, independence, rehabilitation and recovery. Work in an integrated and partnership way with primary, secondary, social care, the independent and voluntary sector, and others to improve the health and care of patients.
KEY RESULT AREAS: Core duties and responsibilities:
1. Responsible for delivering skilled nursing care to patients in the community setting e.g. recording and monitoring a patients clinical observations, wound care management, medicines management including screening and vaccination and immunisations.
2. Participate in the planning and delivery of packages of care, including liaison with other health professionals and statutory and voluntary agencies to deliver skilled nursing care and support any changes.
3. Responsible for the liaison between patients/carers and other agencies which includes arranging and coordinating multi-disciplinary team meetings to evaluate patient care and ensure effective use and timely transfers from the service.
4. Demonstrate a range of clinical skills that supports high quality person-centered care for the caseload population working autonomously in a variety of community settings.
5. Record all patient contacts and activities accurately and contemporaneously maintaining accurate records and statistical returns as required by the Trust including entering data onto the Trust's systems within the required timeframes.
6. To be responsible for assessing telephone enquiries from the general public and residential and nursing home staff leading to giving advice or attending the patient to carry out an assessment and/or treatment.
7. To be a skilled communicator establishing and maintaining excellent relationships with Health and Social Care professionals, initiating referrals where appropriate to ensure adequate support for patients and carers.
8. Work in partnership with individuals, formal and informal carers, and other services to promote the concept of self-care and patient-led care where possible, providing appropriate education and support to maximise the individuals independence and understanding of their condition(s) in achieving their health outcomes.
9. Lead and foster a culture of openness and recognition of duty of candour in which each team member is valued, supported, and developed, inspiring a shared purpose to support the delivery of high-quality effective care.
10. Contribute to the development, collation, monitoring, and evaluation of data relating to service improvement and development, quality assurance, quality improvement and governance, reporting incidents and developments related to community nursing ensuring that learning from these, where appropriate, is disseminated to a wider audience to improve patient care.
11. Work within the clinical governance framework to maintain consistent high standards of care, working from an evidence base and current best practice.
12. To be a link nurse for the team in identified areas e.g., continence and infection control.
13. To be aware of and implement health and safety policies which includes completing incident forms and participating in investigation of incidents as appropriate.
14. Contribute to the development of and implementation of policies, procedures, and guidelines relevant to own area of work.
15. Act as a mentor for students and participate in all aspects of and levels of training in order to consolidate theoretical and practical teaching of staff and learners.
16. To participate in Clinical Supervision within the team on a regular basis.
COMMUNICATION WITH OTHERS: Establish and maintain effective communication with patients, relatives, and carers in complex and potentially challenging situations.
