The post is 37.5 hours per week, Monday to Friday and initially is a fixed term contract for 18 months. Following the induction period, you will be working within the community and travelling across the catchment area, which is quite extensive. The Droitwich, Ombersley, and the Rurals Neighbourhood Mental Health Team has grown significantly since the NHS England Mental Health Transformation model was implemented in 2021, where mental health services have been situated in the heart of the community that they work with. Main duties of the job Work as a key member and resource of the multidisciplinary (MDT) core team. Aspects of the role would also include close working within the MDT, undertaking and supporting service users accessing the care of the team for recovery-focused interventions. There is scope for professional development as the Trust has a repertoire of training available to those wishing to extend their skill set. Support registered professionals to deliver evidenced based interventions, managing a caseload of patients. Supporting and signposting to community-based services to improve social isolation and mental wellbeing. You will have experience of working with individuals who have mental health difficulties. The ability to work flexibly as the needs of the service demands is essential. Attend weekly MDT meetings, participate in professionals meetings, refer and signpost to appropriate services, dependent on patient need to improve overall recovery journey. Provide support and guidance to the carers of patients accessing the services.
