Job summary Gateshead Inner West are seeking a highly skilled and experienced nurse to join our dedicated PCN Community Nursing Team. You will play a critical role in delivering high-quality, patient-centred care to individuals within the community setting. You will utilise your advanced clinical skills to assess, diagnose, treat, and manage complex cases, working collaboratively with other healthcare professionals to ensure a holistic approach to patient care. This role provides an opportunity for patients to avoid the need to be referred to a GP, thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment. Main duties of the job Responsible for the assessment, planning, implementation and evaluation of care, without direct supervision, to patients, carers and relatives within their home environment, enabling them to enjoy maximum independence and optimum quality of life.
