Caerphilly County Borough Council is seeking an experienced and dedicated Social Worker to join our 16 Plus Team. This is a fantastic opportunity to work with young people transitioning into adulthood, ensuring they receive the right support to achieve independence and stability.
You will play a key role in supporting care-experienced young people, providing high-quality assessment, care planning, and intervention. The role involves working closely with multi-agency partners, advocating for young people, and ensuring they have access to education, training, housing, and emotional well-being support.
Responsibilities
Provide high-quality assessment and care management for young people requiring ongoing intervention.
Review care plans within statutory timescales, ensuring they align with the young person's needs and aspirations.
Work in partnership with education, housing, health, and support services to promote positive outcomes.
Offer guidance and practical support to young people, empowering them to achieve independence.
Represent the service at court hearings, panel meetings, and multi-agency forums.
Ensure effective multi-disciplinary collaboration with partner agencies.
Contribute to service development initiatives to enhance outcomes for young people.
