We are ideally looking for Clinical Pharmacists who have already completed the CPPE Pathway .
This is an exciting opportunity to join our existing team. Professional Leadership will be provided by the Consultant Pharmacist at OHP and the Lead Clinical Pharmacist in the PCN.

Main duties of the job
The position of Primary Care Network Clinical Pharmacist has an essential role in improving the healthcare we provide to our patients. We are recruiting for Clinical Pharmacists to work with existing practice and PCN teams to deliver consistency and excellence in medicines optimisation across the member practices.
We are looking for Clinical Pharmacists who will work as part of amulti-disciplinary team, developing and managing medicines optimisationservices within our practices. Demonstrating improvements in value and patientoutcomes and contributing to reducing health inequalities through the safe andappropriate use of medicines. This includes implementation of systematic processesfor improving medicines optimisation across our PCNs and the wider OHP network.Work programmes will be prioritised according to the population needs of thePCN.

Job responsibilities
The post holder is a Clinical Pharmacist, who acts within their professional boundaries, supporting and working alongside a team of Health Care Practitioners within general practice and a Primary Care Network (PCN).
The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will perform medicines reconciliation on transfer of care and implement systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the PCN.
