The Medical Division at Mid Yorkshire Teaching NHS Trust provides Acute care across the range of medical specialities in a large modern purpose-built hospital in Wakefield, which includes a large Acute Assessment Unit, Acute Elderly Assessment Unit, Coronary Care and Acute (Respiratory) Care Unit. We also provide care for Acute Elderly Admissions and step-down care for patients in a less acute phase of their admission at Dewsbury & District Hospital.
Successful applicants will support the Elderly specialty team in the management of their in-patient & outpatient workload, and will participate in the Trust-wide out-of-hours cover across both hospital sites.
Our Elderly Medicine team provides a crucial role for the Trust and our local community, by striving to achieve excellent patient care and experience every time for all our patients. The team provides care for approximately 250 inpatient beds spread across 2 sites and cares for the frailest and most vulnerable in the society, as well as supporting other medical colleagues in caring for older patients. We have grown our team to be able to run dedicated Elderly Care out of hours roster separate to general medicine.

Main duties of the job
The main duties of the post will include:
1. Provision of care with responsibility for the management of patients with acute and chronic conditions including complex cases.
2. Referral of patients to other specialities as appropriate.
3. Out-patient activities - clinics.
4. Involvement in specialty and cross-specialty multi-disciplinary teams and meetings.
5. Involvement in relevant administration duties.
6. Teaching and supervision of junior medical staff and other clinicians as appropriate.
7. Active involvement in audit and clinical governance including service improvement activities.

We run a broad number of specialist areas across the trust as follows:
On our Wakefield (Pinderfields) site:
1. A 40 bedded acute frailty unit (ACE) supported by a Rapid Elderly Assessment Care Team (REACT) focused on comprehensive geriatric assessment (CGA), with a view to getting patients home in a very timely fashion.
2. A 40 bedded Orthogeriatric unit caring for frail patients following fracture neck of femur and aiming for rehabilitation transfer or leaving hospital within 5 days - the vast majority of patients now meeting best practice standards and operated upon within 36 hours of admission to our Emergency Department.
3. A 40 bedded base ward with consultants having specialist interest in nutrition, Parkinsons disease and falls.
On our Dewsbury site:
1. A 20 bedded acute frailty unit (ACE) supported by the acute frailty team, providing CGA assessment and early discharge.
2. A 26 bedded unit which we are in the process of converting to a short stay unit.
3. Three 28 bedded elderly care wards, one taking post-operative fracture neck of femur patients for rehabilitation.
We also run a large Movement Disorder Service seeing all patients over 75 with movement disorder problems with 5 clinics per week across the Trust supported by Parkinson disease nurse specialists. We also run a wide range of clinics and provide home visits to the local community.
The Service Role
From a clinical perspective these posts will participate in the cross site senior trainee rota. The senior trainee rota is compliant with the 2016 junior doctor contract averaging 48 hours per week. The illustrative rota pattern is attached to this job description.
Duties will be based primarily on our Pinderfields Hospital site although all our medical staff are involved in delivering care to patients on all our sites.

* Have evidence of achievement of foundation competences from a UKFPO-affiliated foundation programme or equivalent, by time of application in line with GMC standards/Good Medical Practice.
* Have evidence of achievement of Internal Medicine Stage 1 capabilities, for the round of application, via one of the following methods:
* Current employment in a programme which leads to successful completion of Internal Medicine Stage 1 Training by the advertised post start date, via one of these approved routes:
* UK Internal Medicine Stage 1 Training.
* UK ACCS (Internal Medicine).
* UK Broad Based Training (medicine route).
* A standalone UK IMY3 programme (following completion of UK core medical training/ACCS (acute medicine)).
* JRCPTB internationally level 3 accredited equivalent Internal Medicine Stage 1 Training programme.
* Successful completion of one of the programmes listed above, evidenced by ARCP.
* Completion of UK ST3 in General Internal Medicine and satisfactory progress for any subsequent GIM training, by the advertised post start date, evidenced by ARCP.
