Main duties of the job
1. To assist registered nurses in caring for patients on the day of discharge from hospital.
2. To provide all aspects of care following ward care plans.
3. To provide nutrition and diet.
4. To ensure that all patients are comfortable and reassured.
5. To answer the telephone and relay messages to the Ward Coordinator.
6. To communicate all care and concerns to the relevant nurse.
7. Monitor observations (training will be provided).
Detailed job description and main responsibilities
1. To meet and greet patients on arrival to the ward area and make them comfortable in chair/bed.
2. To provide personal care when required.
3. Monitor vital signs, responsible for checking and reporting any changes to Registered nurse.
4. Assisting with mobility to the bathroom and patients who are bed bound.
5. Assisting with nutrition and fluids following instructions from registered nurse, ensuring patients receive correct dietary requirements or special diets.
6. Maintaining a clean and safe environment at all times following infection control guidelines.
7. Supporting patient comfort and well-being engaging in conversations and offering emotional support when needed.
8. Accurately record all patient care observations and interactions.
9. Working as a team supporting a multi-disciplinary team setting.
